Home visits

If a patient is medically unfit to attend surgery and requires a home visit, we need:

  • Details and symptoms of what is wrong with the patient.

This information enables GP’s to prioritise their workload to ensure patients receive the best treatment for their needs.

A community nurse or our home-visiting paramedic may visit on behalf of the doctor.

It is helpful for home visit request to be made as soon after 8am as possible.
